Blue Slag Glass Creamer.
I do not know the maker.  Could be Sowerby or Davidson, or something else.
Overall in very good condition in my opinion.  Minor maker flaws common with antique glass. May have a couple of very minor nicks on rim.  Difficult to tell.  Some minor discoloration on handle.
Approx. 3 and 1/2 inches tall.
I take close up photos and feel they describe the item better than any description I can provide.
